Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

We are happy to offer a 5% prepayment courtesy for all treatment paid in full at the time of treatment (cash or check only). For your convenience, we accept Visa, Mastercard and Discover. For patients who desire a monthly payment plan, arrangements may be provided. Our office manager will assist you in filling out the proper application and approval is provided quickly.

   
Dental Insurance

We accept many types of insurances including Blue Cross, Valuecare, Delta, etc. Please contact us by phone if you have billing questions. We look forward to hearing from you.
